Organizations request your Certificate of Insurance as proof of your coverage in instances like loan applications or equipment rental—any situation where a third party may end up invested in what happens at your property, whether that’s offering a loan or entrusting your house with leased office equipment. You’ll also likely be asked for a Certificate of Insurance whenever you host an event at an off-site venue.

We require a minimum of two weeks’ notice prior to the event for any Certificate of Insurance or Event Review requests. If you are inside of the two week window, we cannot guarantee Certificate requests to be completed before your event.

Note: Before you sign a contract with a venue, submit it to us so that we can confirm you’re in compliance. Make sure that you complete this form accurately and upload the contract so that we can keep the process moving swiftly.

Failure to accurately complete this form or provide the contract will result in a delay in processing.
