Organizations request your Certificate of Insurance as proof of your coverage in instances like loan applications or equipment rental—any situation where a third party may end up invested in what happens at your property, whether that’s offering a loan or entrusting your house with leased office equipment. You’ll also likely be asked for a Certificate of Insurance whenever you host an event at an off-site venue.

We require a minimum of two weeks’ notice prior to the event for any Certificate of Insurance or Event Review requests. If you are inside of the two-week window, please complete the form as soon as possible, and we will do our best to process your request; however, we cannot guarantee your request will be completed before your event. Please keep in mind contacting MJ or emailing will cause a delay in processing. 

Note: Before you sign a contract with a venue, submit it to us so that we can confirm you’re in compliance. Make sure that you complete this form accurately and upload the contract so that we can keep the process moving swiftly.
